Upon acceptance, you will be provided with a Tuberculosis (TB) Medical Report Form that is required for all admissions. Those who do not complete this form by the given date will not be eligible for enrolment. 

Once complete, please mail or email the above documents accompanied by your most recent Diploma, School Transcript, and Proof of I.D. to the NuWave address below.

For students who are self-funded, we will only accept forms of payment via bank draft, money order, or cheque.
